Dietary Restrictions / Physical Restriction

If you have a dietary /allergy /physical restriction, check the appropriate box on the registration form and provide a complete description of your restriction.

Dress

Business casual attire is appropriate for all meetings and networking receptions. The DIANA and Nexus Awards Reception, Banquet and Celebration on Monday, June 11 call for business or cocktail attire.

Emergency Contact

In the event of a medical or other emergency, HDMA staff and appropriate emergency personnel can respond more effectively and efficiently with appropriate emergency contact information. Indicate the name, relationship and cell number for your emergency contact on the conference registration form. Note that emergency contact information is for emergency conference use only.
