Registration Information
The registration fee includes all Seminar materials, lunch and reception on Wednesday, September 30, breakfast, refreshment breaks and boxed lunch on Thursday, October 1.

Cancellations
Registration is transferable to another individual from the same company only. Individual cancellations must be postmarked or faxed to HDMA by Friday, August 28, 2009 for a partial refund. A $200 processing fee applies to individual cancellations sent by Friday, August 28, 2009. No refunds will be issued for cancellations after Friday, August 28, 2009.

Reservations
Reservation must be made by Wednesday, September 9, 2009. Reservations made after the cut-off date will be subject to space and rate availability. Reference the Healthcare Distribution Management Association (HDMA) to receive the special $119 room rate (subject to applicable state and local taxes).
Hotel Cancellation Policy
All changes and cancellations should be communicated directly to the hotel. Rooms cancelled less than 48 hours prior to the scheduled arrival date are subject to a cancellation fee equivalent to the cost of one night, plus applicable taxes.

$20 per day for self parking with no in/out privileges. $28 per day for indoor valet parking with in/out privileges. $28 per day for oversize vehicle parking. Parking is on first come-first serve basis. (The height of the garage is 6'2" - for vehicles larger than 6'2" there is parking across the street at comparable rates.)
